Medical Benefits

There is one (1) type of medical plan at Glendale Adventist Medical Center; we have a self-insured plan that covers employees for a wide range of medical services.

 

  • There is a $20 co-payment for office visits.
  • For inpatient and outpatient hospitalization, the plan will pay 100% if services are performed at GAMC or WMMC
  • Services performed at any Adventist facility, the plan will pay 90% and employee pays 10%.
  • Services performed at any non-Adventist facility, the plan will pay 50% and employee pays 50%.

Dental Plans

Adventist Health PPO Dental Plan
Allows you to select any dental provider of your choice. Coverage includes:

  • Preventative Services covered at 100%, two (2) per calendar year
  • Basic Services covered at 80%
  • Major Services covered at 50%

Cigna Dental HMO Plan
Requires you to select a dental provider from their contracted panel. A schedule of benefits is available upon request.

Safeguard Dental HMO Plan
Requires you to select a dental provider from their contracted panel. A schedule of benefits is available upon request.

Vision Care

Vision care is included with your medical plan. The vision plan pays 80% up to a $300 maximum for a two (2) year period.

Prescriptions

Prescriptions are processed through Express Scripts. The co-payments are as follows:

  • Generic: $10
  • Brand: $25

Mail Order - must be used for all maintenance medications. Three months for the cost two.

Retirement Plan

Employer contributes 3%of your annual salary

401(a) Qualified Plan-Matching Contribution Employer
Matches $0.50 for each dollar you contribute, up to the first 4% of your annual salary

403(b) Tax Sheltered Annuity Plan

  • Employees under 50 years of age by the year's end may contribute the lower of $16,500 or 100% of compensation.
  • Employees 50 years of age and older by the year's end may contribute the lower of $22,500 or 100% of compensation.
  • Twenty-six (26) funds to select from.

Employee Assistance Program

The EAP at Glendale Adventist Medical Center provides you and your family with guidance, focus and support for a wide range of personal and work-related problems that may otherwise interfere with your daily lives. Six (6) free sessions are available to each family member per year.

A voluntary life insurance plan is also available.

Accident Insurance

The policy has aprovision for accidental death or dismemberment (AD&D). In the case of accidental death, the policy would pay the designated beneficiary twice the maount of life insurance listed above. A voluntary plan is also available.

Childcare

On-site childcare is available for employees at a discounted rate and can be paid through payroll deduction.

Paid Leave Policy

Paid leave is available for employees who work at least 40 hours per two-week pay period. Accrual is based on years of service and hours worked in a pay period.

Official Holidays

Employees who work on the official holidays will be paid a holiday premium.

Tuition Assistance

Tuition assistance is available for full-time employees who have been employed at GAMC for a minimum of one year. An approval process is in place for this benefit.

Alternative Transportation, Commuter Program

Glendale Adventist Medical Center provides transportation subsidies to those employees who take the bus or travel on the Metrolink. Please contact Human Resources at 818-409-8040 for details.
